Your disability benefits are specific to your employer. Please get in touch with your Human Resources department for exact details. If you need a note for time off work from one of our doctors, please know what dates you need off and send an email or emessage (through the portal). A note will be sent back to you.
If you’ve been directed to file for disability through the state, please go to: http://www.edd.ca.gov/Disability/ and register and file a claim. After submitting a claim, you will be given a receipt number. Please send that to our office in an email, and we will fill out the physician’s side within 1-2 weeks.
