Higher Maternal Vitamin D Levels in Pregnancy Linked to Better Cognitive Outcomes in Children

A healthy pregnancy is about much more than a smooth delivery—it’s also about laying the foundation for a child’s lifelong health. Increasingly, research is showing that what happens during pregnancy has lasting effects on a child’s growth, brain development, and future well-being.

One nutrient that continues to garner attention is vitamin D. Long known for its role in bone health and immune function, vitamin D may also be crucial for brain development. A recent study published in The American Journal of Clinical Nutrition and highlighted by Contemporary OB/GYN adds weight to this idea, showing that higher maternal vitamin D levels during pregnancy are associated with better cognitive performance in children.

At our practice, we’ve long recognized the importance of vitamin D. That’s why we screen all pregnant patients for vitamin D deficiency at the time of their first prenatal labs. When deficiency is identified, we recommend appropriate supplementation, ensuring mothers and their babies start off with the best possible foundation.


The Study: Vitamin D Levels and Child Cognition

The research team set out to evaluate whether a mother’s vitamin D status during pregnancy had measurable effects on her child’s later cognitive abilities. Vitamin D was assessed through blood levels of 25-hydroxyvitamin D [25(OH)D], the best marker of vitamin D status. Children’s cognition was measured using standardized cognitive performance assessments.

Here’s what they found:

  • For every 10-ng/mL increase in maternal vitamin D, children had:
    • A 1.11-point higher overall cognition score
    • A 1.21-point higher fluid cognition score
  • No statistically significant effect was observed on crystallized cognition (skills such as vocabulary and general knowledge).
  • While results did not differ significantly by race, the positive association appeared stronger in children of Black mothers.

These findings suggest that maternal vitamin D sufficiency may be particularly beneficial for fluid cognition—the brain’s ability to reason, solve new problems, and adapt to unfamiliar situations.


Fluid vs. Crystallized Cognition

Cognition isn’t one-size-fits-all. Researchers typically divide it into two broad categories:

  • Fluid cognition: The ability to think quickly, solve novel problems, analyze new information, and apply logic in unfamiliar settings. This type of intelligence is thought to peak in early adulthood.
  • Crystallized cognition: Knowledge and skills acquired over time, including vocabulary, cultural knowledge, and learned facts. This typically builds across a lifetime and depends more on educational and environmental exposure.

The study found a stronger relationship between maternal vitamin D and fluid cognition than crystallized cognition. This makes sense, as fluid cognition is more closely tied to brain development and neurological wiring, processes heavily influenced by prenatal nutrition.


Why Vitamin D Matters in Pregnancy

Vitamin D is often called the “sunshine vitamin,” since our skin produces it in response to sunlight. But modern lifestyles, limited sun exposure, higher skin pigmentation, and geographic location can all reduce vitamin D synthesis.

During pregnancy, vitamin D plays multiple roles:

  • Bone development: Supports calcium absorption and skeletal growth in the fetus.
  • Immune regulation: Helps the mother’s immune system tolerate pregnancy and may reduce risks of preeclampsia and gestational diabetes.
  • Brain development: Vitamin D receptors are present in fetal brain tissue, suggesting a direct role in cognitive development.

Deficiency in pregnancy is common. Studies estimate that 20–50% of pregnant women in the U.S. are vitamin D deficient, with even higher rates among women with darker skin, those living in northern latitudes, and those with limited sun exposure.


Our Approach to Vitamin D Screening and Supplementation

Given these risks, we have always prioritized vitamin D as part of routine prenatal care. At the time of the first prenatal labs, we check vitamin D levels in every patient. This proactive approach allows us to identify deficiency early and recommend supplementation tailored to the individual’s needs.

  • Mild deficiency may be corrected with standard prenatal vitamins plus an additional supplement.
  • Moderate to severe deficiency often requires higher doses under medical supervision, followed by repeat testing to ensure adequacy.

By identifying and treating deficiency early, we aim to optimize both maternal health and fetal development. The latest findings about cognitive benefits reinforce the importance of this longstanding practice.


Implications for Patients

This study doesn’t prove that vitamin D alone determines a child’s cognitive abilities—many factors, from genetics to early childhood environment, play major roles. However, it adds compelling evidence that maternal nutrition is one piece of the puzzle that parents and providers can influence.

  • Balance sun exposure and safety: Moderate sunlight helps vitamin D production, but overexposure increases skin cancer risk.
  • Eat vitamin D–rich foods: Fatty fish (like salmon and sardines), fortified dairy products, and eggs can contribute to intake.

Most importantly, individual needs vary, so supplementation should be guided by your healthcare provider rather than one-size-fits-all advice.


The link between higher maternal vitamin D levels in pregnancy and improved child cognition is another reminder that prenatal care sets the stage for a child’s future health. While no single nutrient guarantees better outcomes, vitamin D is a modifiable factor with benefits that extend beyond pregnancy.
